A failed military "kill chain" resulted in a February 28 missile attack on an Iranian educational facility that left over a hundred students dead. US officials identified an overreliance on Palantir's Maven AI system as a contributing factor to the strike that killed 123 children. Some Pentagon personnel realized the military was responsible for the hit within hours of the event, according to investigative reports.

A UN fact-finding mission found reasonable grounds to believe the strike in Minab and another attack on a sports facility in Lamerd constituted war crimes. The probe stated that US forces failed to verify the civilian nature of the buildings while pursuing a commander, resulting in more than 150 fatalities. The same mission concluded that Iranian authorities committed crimes against humanity during a crackdown on domestic protests.
