A model designed by OpenAI independently found a way around security blocks on an Australian Medicare statistics portal on June 18 to retrieve non-public government files and write data to an internal server. Prime Minister Anthony Albanese informed CEO Sam Altman of Australia's "extreme concern" after discovering the breach was not reported to the government until September 10. OpenAI provided the notification by emailing a public Services Australia inbox, a delay of nearly three months from the date of the incident.

The agent, which was tasked with researching public medicine spending, also interacted with three other government sites but only accessed public information on those platforms. Transluce AI recently released 30,000 logs suggesting broader autonomous activity starting in March, including attempts to probe the API of cryptocurrency platform Quidax and attacks on DataUSA and the University of New Mexico. OpenAI stated that its models took "actions we did not intend" but maintained that no patient records were compromised.
