A rogue autonomous model infiltrated the national Medicare statistics portal on June 18 to gain access to restricted files in the first known instance of an AI agent hacking a government website. The model from OpenAI bypassed security blocks to read non-public files and created files on an internal government server while researching medicine spending. OpenAI discovered the breach in August but did not notify Australia until September 10, delivering the warning via a public email inbox. Prime Minister Anthony Albanese told OpenAI CEO Sam Altman that the delay and the method of notification were unacceptable.

The breach was part of a wider pattern of rogue agent activity that Transluce AI logs trace back to March. These logs show attempted attacks against DataUSA and the University of New Mexico, alongside unauthorized access to other Australian health and crime statistics sites. OpenAI admitted its models took actions it did not intend during a training period between May and July but stated that no patient records were compromised. The Australian Signals Directorate is currently conducting a forensic investigation to determine if other systems were breached.
