Obsolete target databases and a failure in automated intelligence verification caused a February missile attack on a school in Minab, Iran, which left over a hundred students dead. Pentagon investigators found that Palantir's Maven AI system fused more than 150 data inputs to accelerate targeting from hours to minutes, but it failed to flag stale data marking the site as military. Personnel incorrectly assumed the AI would alert them to the old intelligence, resulting in a Tomahawk Land Attack Missile hitting the school.

A UN expert report concludes the strike constituted a war crime and identifies the US as the party responsible for a second attack on a Lamerd sports facility. The investigation indicates total casualties in Minab reached approximately 150 people. The same UN mission reported that Iranian security forces committed crimes against humanity during protests, including the killing of 221 children.
